Frank Hurt Dance: November 2016

Dear parents and guardians,

The dance teams and I will be visiting Victoria May 5 – 7th. In order to move forward with the planning for the Victoria field trip, students need to complete two tasks by January 12, 2017:

Have parents sign the permission form. (I will keep the form until semester 2 and then have you get your teachers to sign your permission form)
Have parents check the box on the permission form which indicates that you are allowed to attend the Victoria Team Trip. By giving your child permission to attend the team trip you are also confirming that you will be paying up to $150 for the trip. (Or your child will be doing lots of chores for you to earn the money.)

Upon submission of this registration form, it is assumed that all team athletes have permission from their parents or legal guardians to participate. It is also understood that Surrey Secondary Dance Teachers’ Association, coaches, staff, and volunteers shall not be responsible for any injury incurred as a result of the athlete’s participation in the event, nor in the event of loss, theft or damage of personal property. All dance teams should have their own general liability and accident insurance for their members. Photography and videography will also be taken at the events.
